Olmsted County helps people with disabilities of all ages find and access the support they need to live, work, and be part of their communities. The county provides short-term planning and ongoing support to make sure people have the tools to live independently and get the support they need. Our goal is to help people and their families access the resources that can improve their lives.
How to get started: Get a MnCHOICES Assessment
To schedule your assessment, submit the online referral form or call 507-328-6519.
Next, a social worker will contact you and ask you questions to provide information about potential options depending on your age and needs.
Services for children and adults with disabilities
If you're a parent or guardian of a child with a disability, the MnCHOICES Assessment can help you access the right services for your child, such as therapy, educational support, and family resources. The goal is to provide your child with the support they need to grow, develop, and live independently.
Adults with disabilities
For adults with disabilities, the MnCHOICES Assessment helps identify services that can support independence and daily living. This process ensures that you get the right resources to maintain a quality of life and live as independently as possible.
Each program listed below has some very specific eligibility criteria for individuals under age 65 who have a disability.

Services for seniors
Seniors with disabilities can benefit from the MnCHOICES Assessment by accessing services that help them remain in their homes and communities longer, and avoid unnecessary institutional placements.
