An establishment that stores, prepares packages, serves, vends, or otherwise provides food and/or beverage for human consumption. A Food Establishment license includes the ability to provide catering and commissary services. |
| Limited Food Establishment | $700 | A food establishment that provides one or more of the following: a. prepackaged food that receives heat treatment and is served in the package; b. frozen pizza that is heated and served; c. a continental breakfast such as rolls, coffee, juice, milk and cold cereal; d. any beverage such as soft drinks, coffee, alcoholic or nonalcoholic drinks; e. cleaning for eating, drinking, or cooking utensils when the only food served is prepared off site; or f. primarily provides beverages that consist of combining dry mixes and water or ice for immediate service to the consumer. Must use equipment and utensils that are nontoxic, durable, and retain their characteristic qualities under normal use conditions. |
| Caterer / Commissary | $700 | A caterer prepares, transports, or serves food to a location other than the Commissary for immediate consumption. A commissary is a food establishment where food, containers, or supplies are kept, handled, prepared, packaged, and stored. |
| Seasonal Food Establishment | $400 | A food establishment which is closed five (5) consecutive months of the year or more. A Seasonal Food Establishment license includes the ability to provide catering and commissary services. |
| Seasonal Temporary Food Stand | $331 | A food establishment that is disassembled and moved from location to location, remaining no more than twenty-one (21) consecutive days in one place. |
| Mobile Food Unit | $331 | A food establishment that is a vehicle-mounted unit either motorized or trailered, operating no more than twenty-one (21) consecutive days annually at any one (1) place. |
| Food Cart | $331 | A non-motorized vehicle self-propelled by the operator. Food Cart shall be synonymous with "Push Cart." |

| School Concession Stand | $331 | A food and beverage service establishment located in a school, on school grounds, or within a school-owned athletic complex, that is operated in conjunction with school-sponsored events. A school kitchen or school cafeteria is not a school concession stand. |
| Special Event Food Stand | $30 | A food and beverage service establishment that is used in conjunction with celebrations and special events, and which operates for no more than ten (10) total days in the current calendar year. |
| Special Event Mobile Food Unit or Cart | $50 | A mobile food unit or cart that is used in conjunction with celebrations and special events, and which operates for no more than ten (10) total days in the current calendar year. |

| Day Care Center with Food Prep | $600 | Any child-care facility which meets the requirements for licensing by the Minnesota Department of Human Services under Minnesota Rules Chapter 9503 which provides care of six or more children for periods of more than 3 hours but less than 24 hours. The term includes, but is not limited to, nursery schools, hospital centers, after school programs, and child day care centers. Day Care Center with Food Prep: A day care facility where food is prepared and served on-site. |
| Day Care Center with Limited Food Prep | $350 | Day Care Limited - A daycare facility where food preparation is limited to snacks or catered foods only. On-site preparation of potentially hazardous foods as defined in the Minnesota Food Code (4626.0020 subp 62) is not permitted. Snacks are foods that require limited, on-site food handling or service including, but not limited to, fruits, cereals, prepackaged bakery products, candy and chips. Catered foods must not require additional preparation or heating at the day care center. |
| Bed and Breakfast | $180 | A private home where sleeping accommodations are offered for one or more nights for transient use to no more than eighteen (18) persons and includes a single meal per day served on the premises. |
| Boarding | $331 | A food and beverage service establishment where food or beverages, or both, are furnished to five or more regular boarders, whether with or without sleeping accommodations, for periods of one week or more. |
| Manufactured Home Park (MHP) | $300 base fee, plus $10 per site. | Any site, lot, field or tract of land upon which two or more occupied manufactured homes are located, either free of charge or for compensation, and includes any building, structure, tent, vehicle or enclosure used or intended for use as part of the equipment of the manufactured home park. |

Year Round: $300 base fee, plus $10 per site. Seasonal (closed for 5 consecutive months): $150 base fee, plus $5 per site. |
Any area, whether privately or publicly owned, used on a daily, nightly, weekly, or longer basis for the accommodation of five or more tents or recreational camping vehicles free of charge or for compensation. |
| Lodging - Camper Cabin | $10 | A permanent rustic enclosure with walls and a floor that does not include a kitchen or bath; is located in a state park administered by the commissioner of natural resources, at a resort as defined under section 157.15, subdivision 11, or at a recreational camping area as defined under section 327.14, subdivision 8; and is intended to be a place where sleeping accommodations are furnished to the public. |

| Establishment Late Fee | 5% for the first month or partial month late, an additional 10% for each additional month or partial month late. | A late fee will be assessed if payment, signed renewal form and completed worker’s compensation insurance form are not received by the due date. |
| Special Event / Mobile Food Unit Late Fee | $30 | A late fee will be assessed if the application is received three calendar days or less before the event. |
