Health Care Assistance Programs - Medical Assistance and MinnesotaCare

If you qualify, Medical Assistance and MinnesotaCare can provide low-cost or no-cost health insurance. The Minnesota Department of Human Services manages these two government-sponsored programs.

It is important you complete your renewal paperwork on time to keep your health insurance. Determine when to watch for your renewal paperwork in the mail.

Federal Medicaid (Medical Assistance) changes

Changes to federal Medicaid law will affect some Medical Assistance (MA) members in the coming years. Minnesota Department of Human Services will provide the most up-to-date information on what is changing, who may be affected, key dates, and any steps members need to take.

Find out if you qualify

If you have Medical Assistance or MinnesotaCare

If your income has changed, your coverage might have changed. We need to know about any changes to your income to make sure you get the correct coverage.

If you do not have Medical Assistance or MinnesotaCare

If you have lost or will lose your health insurance, you might qualify for a Special Enrollment Period (SEP). Other qualifying life events might also let you use the special enrollment period. Find out more information on the MNSure Special Enrollment Period (SEP) web page.

To apply online

Gather your information

  • Social security numbers or other document numbers
  • Employer and income information for everyone in your family. (for example, from pay stubs, W2 forms, or wage and tax statements)
  • Policy numbers for any current health insurance
  • Information about any job-related health insurance available to your family

Tip:  Download and print the MNSure Application for Health Coverage and Help Paying Costs document. The document contains the entire application, along with step-by-step instructions. You can use it as a guide for gathering all the necessary information before filling out the online application.

The online application is fast and easy! You may be able to make real-time decisions using the online application. You can also get help online if you have questions during the application process.

How to Submit Documents

You can send us documents or proofs through our secure portal MNBenefits Upload Documents , drop box, or mail to 2117 Campus Drive SE Suite 100, Rochester, MN 55904, or fax 507-328-7956.
