Intellectual and Developmental Disabilities

Olmsted County offers case management services and support for people for children and adults with developmental disabilities (DD), which is also known as Rule 185 Case Management. Case managers help people in accessing necessary social, medical, educational, and other supports. They work closely with individuals to identify their goals and needs, ensuring ongoing support and services are in place to help achieve those outcomes.

Eligibility

To be eligible for Rule 185 case management, a comprehensive diagnostic evaluation must indicate that the person has a diagnosis of developmental disabilities or related conditions. A child younger than age five without a conclusive diagnosis must demonstrate significant sub-average intellectual function concurrent with demonstrated deficits in adaptive behavior.

Services for children and adults

Case managers help individuals find resources for job training, life skills, housing, and more, and provide support for individuals with intellectual or developmental disabilities.

How to apply?

Step 1: Call our intake team at 507-328-6519 or email HCBSintake@olmstedcounty.gov to discuss service needs and provide information regarding your DD Case Management request.

Step 2: An application will be sent electronically or mailed to you for completion. Please complete all forms on the application. To reduce processing time, please include all required assessments as indicated on the application. 

Step 3: Submit
Submit  completed application to HCBSintake@olmstedcounty.gov

or via fax to: 507-328-6519

Step 4: A referral will be made to the Olmsted County Assessment Team for a MnCHOICES Assessment. An intake worker will call to inquire about additional information. Once assigned, an assessor will meet with you to complete the assessment. This assessment also assists in determining eligibility for supports and services.

Step 5: The application will be reviewed with the eligibility team. During this review, it may be determined that additional testing is required. You will be contacted by a member of our intake team for next steps. Please note: Olmsted County will hold your service request open for 90 days. If a completed application is not received within 90 days and no communication has been received regarding the status of your application, your request will be closed.

What is a developmental disability?

Developmental Disability is a diagnosis characterized by substantial functional limitations and significantly sub-average intellectual functioning, full scale IQ or 70 or less, and exists concurrently with demonstrated deficits in adaptive behavior. All conditions are manifested before the persons 22nd birthday.

Related Conditions

A related condition is a diagnosis of severe, chronic disability that meets all of the following conditions:

  • Is attributed to Cerebral Palsy, epilepsy, autism, Prader-Willi syndrome or any other condition other than mental illness or emotional disturbances.
  • Is found to be closely related to developmental disabilities because the condition results in impairment of general intellectual functioning or adaptive behavior similar to that of people with developmental disabilities and requires treatment or services similar to those required for people with developmental disabilities.
  • Is manifested before the person reaches age 22.
  • Is likely to continue indefinitely.
  • Results in substantial functional limitations in three or more of the following areas of major life activity: self care, understanding and use of language, learning, mobility, self direction, or capacity for independent living.

How is eligibility determined?

Olmsted County's Rule 185 Eligibility Review Team reviews all service requests. The following documents are reviewed:

  • Comprehensive Diagnostic evaluation that includes standardized testing of intellectual functioning and assessments of adaptive behavior within the last three years.
  • Medical evaluation no older than 12 months from the date of application. This evaluation must include an evaluation of the applicant's general physical health, including vision, hearing, physical and or neurological disorders.
  • Diagnosis codes (ICD-10 codes) related to developmental disabilities and/or related conditions.

It may be necessary to pursue additional testing to meet the service eligibility requirements for the state of Minnesota.

Where to get a diagnostic test?

Note: Olmsted County does not endorse these providers, and this list is not exhaustive.

Below are some providers that can help with diagnostic testing for DD case management applications. Please request both IQ and adaptive testing. If applicable, inform the provider that the individual does not communicate with words. Be sure to check with the provider about insurance billing.
