Residents invited to share input on Olmsted County’s health needs
Complete the Community Health Needs Assessment survey by May 31
The Community Health Assessment and Planning (CHAP) team is seeking community feedback to outline pressing health issues in Olmsted County.
Now through May 31, 2025, Olmsted County residents are asked to participate in the Community Health Needs Assessment (CHNA) survey to help prioritize our area’s top health concerns. This process, which takes place every three years, is led by Olmsted County Public Health Services (OCPHS), Mayo Clinic, and Olmsted Medical Center (OMC).
"These surveys help us identify top health issues in our community so we can work collaboratively to make a positive impact on health in Olmsted County. We appreciate your willingness to help identify these health priorities by completing the survey," said OCPHS Director Denise Daniels.
All Olmsted County residents are encouraged to participate in the online prioritization survey. There are over 30 health issues considered, such as health care, environmental factors, financial wellness, chronic diseases, and health behaviors. The survey takes around 10 minutes to complete.
Responses from this survey will help identify community health priorities in Olmsted County. Additionally, the results will help inform community efforts to address these top health issues.
During the last prioritization survey in 2022, residents highlighted mental health, substance use, and access to care as the county’s top health concerns. Below are highlights of work to address these health priorities.
- OCPHS is working with partners to improve care options for LGBTQ+ people.
- Zumbro Valley Health Center is providing more dental care for clients.
- United Way of Olmsted County is spreading the word about its 211 information and referral program, which connects people to local resources, 24/7 and in over 100 languages.
- Rochester Public Schools is piloting a mental health screening program in eight schools.
Participants should submit their survey before May 31 to make their voices heard.
The CHAP team will share survey results and community health priorities in October 2025.
