Online version of Olmsted County COVID-19 Impact Survey now available
Last week, we communicated that the Olmsted County Community Health Assessment and Planning (CHAP) COVID-19 Impact Survey was being mailed randomly to 2,000 households in Olmsted County. For those who did not receive the paper version of the CHAP COVID-19 Impact Survey by mail who would still like to participate in it, an online version of the survey is now available. The online survey is being offered in three languages: English, Somali, and Spanish.
Information gathered from this survey will help community leaders understand the greater impacts the COVID-19 pandemic has had on Olmsted County residents. It will also help establish community initiatives that address the current three health priorities of mental health, financial stress, and substance use.
The survey takes an estimated 15 minutes to complete and asks questions related to clinical care, the physical environment, personal behaviors, and other factors that affect our health. To get the best and most accurate information and data about our community, we rely on people completing the survey in a timely manner. Participation in the survey is completely voluntary and all answers are strictly confidential; no identifying information will ever be linked to any of the responses. Surveys must be completed by 5 p.m. on Sunday, February 28, 2021.
If you received the COVID-19 Impact Survey in the mail, please complete the paper version of the survey rather than the online survey.

Additional information about CHAP
CHAP is a collaborative effort that began in 2012 and continues to be led by Olmsted County Public Health Services, Olmsted Medical Center, and Mayo Clinic. In addition to these lead agencies, there are numerous community organizations that contribute to the overall process and are committed to keeping our community healthy.
