Apply for a new passport
Adult - New passport
For first time passports, or renewal of a passport expired more than 5 years.
Read all these steps first!
Read the following instructions all the way through before you begin.
1. Get Evidence of U.S. Citizenship
Your evidence of U.S. citizenship must have the original, official seal or stamp of the government office which issued it. You must provide one of the following:
- U.S. Birth Certificate
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship
2. Bring Valid Photo ID
You must present a physical photo Identification (ID) at the time of the application. The most common phot ID we accept is a:
- Fully valid MN Driver's License or Enhanced Driver's License with photo.
If you only have a MN Identification Card/ Instruction Permit or from another State, bring a second photo ID.
Go to travel.state.gov Identification page for the full list of acceptable photo IDs.
3. Download, print, and complete the application below
DS-11: Application for a U.S. Passport
DO NOT print the form on both sides of a single piece of paper (back to back).
The application form must be completed:
- to the best of the applicant's knowledge
- in black ink.
Parental information is required (even if the parents are deceased).
DO NOT SIGN THE FORM.
4. Determine the fees
Fees are based on the age of the applicant and the time in which the applicant needs their passport returned to them.
Bring two (2) forms of payment for each applicant.
- Department of State Fee: Check, money order or cashier’s check payable to: U.S. Department of State – one payment per applicant. (The Department of State does NOT accept cash or credit card)
- Olmsted County Processing Fee: Check, money order, cashier’s check, cash or credit card.
Unacceptable forms of payment:

Stamps, universal and counter checks or "starter checks" (temporary checks normally given to customers who have recently opened a new checking account).
Note that fees are subject to change. See the Olmsted County Passport Fees for more information.
5. Set up an appointment
Use the Online Check-In System to choose a date and time to come into the Olmsted County Government Center to complete the passport acceptance process for your new Passport Book/Card.
Using the Online Check-in System will insure you are able to select a convenient day and time within a 30-day period of time that works for your schedule.

6. What to expect when you arrive
Check in at the Information Desk just inside the front entrance. After you check-in, you will receive further instructions.
If you have questions, there will be staff available to direct you.
When it is your turn, you will be called to the desk.

Minor - New passport
Both parents must be in attendance with applicants under 16 years of age.
We recommend that one parent is in attendance with applicants 16 & 17 years of age.
